Patriot Healthcare partners with First Health Network (previously known as HealthCare Value Management (HCVM)) for our provider network.
Any physician who participates in the First Health (HCVM) Network is considered a Patriot100 Physician. Patriot Healthcare's physician network includes 95% of the physicians in New Hampshire. So, you would be hard pressed to find a doctor that is not part of our network. Please call Patriot Healthcare's Member Support at 1.800.597.7728 prior to any doctor's visit to verify that they are a Patriot100 Physician or click here to search for a physician on the First Health web site. If they are listed here, then they are a Patriot100 physician.
PLEASE NOTE: If you are going in for a physician office visit and the doctor's office is physically located in a Patriot80 hospital, the office visit is considered a Patriot100 facility. You will be covered 100% after you meet your deductible for non-preventive services or 100% before you meet your deductible for preventive care services. Please see your Summary of Benefits for a list of the preventive care services that are covered 100% even before you meet your deductible.
Patriot100 Hospitals and Facilities:
If you have inpatient/outpatient services performed at a Patriot100 hospital or facility, Patriot Healthcare will pay 100% after you meet your deductible. You will not pay any coinsurance for medically necessary services performed at a Patriot100 hospital or facility. Click here for a list of Patriot100 hospitals and facilities.
Patriot80 Hospitals and Facilities:
If you have inpatient/outpatient services performed at a Patriot80 hospital or facility, Patriot Healthcare will pay 80% after you meet your deductible. You will pay 20% coinsurance for medically necessary services performed at a Patriot80 hospital or facility after your meet your deductible. Click here to search for Patriot80 facilities. If the hospital or facility you are looking for is in the First Health directory and is NOT on the Patriot100 list above, then they are a Patriot80 hospital or facility.
Out-of-Network:
If you use an Out-of-Network Provider, there is no negotiated discount. Patriot Healthcare will pay 50% of the "Allowed Charge." The Allowed Charge is equal to the lower of (a) the amount billed by the Provider, (b) the Medicare fee schedule amount for the geographic area, or (c) the usual and customary amount paid for the service. You are responsible for paying the difference between the amount paid by Patriot Healthcare and the amount billed by the Provider. In addition, the maximum Patriot Healthcare will pay for all Out-of-Network services in one year is $100,000 per member.
